Support may include accompaniment to GP, hospital, pharmacy, social, family or community appointments, as well as shopping trips and local errands where these are included in the agreed plan. The pace and level of support should be shaped around the person’s confidence, mobility and preferences.

How we plan safely
Before support begins, TDCARE will discuss needs, preferences, risks, visit routines, communication, family involvement and the desired outcomes. Support should always be appropriate, proportionate and reviewed when needs change.
